What Is Knee Replacement (Knee Arthroplasty)?

Knee replacement surgery (also called knee arthroplasty) involves replacing the worn or damaged parts of your knee joint with artificial implants to relieve pain and restore smooth movement.

At the Northampton Hip & Knee Clinic, we offer a complete range of surgical knee replacement procedures:

  • Total Knee Replacement (TKR / Total Knee Arthroplasty) — replaces the entire joint surface for advanced arthritis or damage
  • Partial Knee Replacement (Unicompartmental Arthroplasty) — replaces only the damaged section of the joint, preserving more natural bone and movement
  • Bilateral Knee Replacement — both knees replaced during the same procedure, if appropriate
  • Minimally Invasive Total Knee Replacement — smaller incision, faster recovery
  • Kneecap Replacement (Resurfacing of the Patella) — for localised kneecap arthritis

Total Knee Replacement

A total knee replacement (TKR) is the most common procedure and is used when two or more compartments are arthritic, or when stiffness, deformity or instability mean a partial replacement would not provide a reliable result. In a TKR, the worn surfaces at the end of the femur and top of the tibia are replaced with metal and polyethylene components. In selected cases, the underside of the kneecap is resurfaced as well.

Modern implants are durable and designed to produce smooth, stable movement. Techniques such as computer navigation or patient-specific instrumentation help improve alignment accuracy. A TKR is usually recommended when arthritis is widespread, when ligaments are weak, when there is significant deformity or when inflammatory arthritis is present. It provides strong, predictable long-term pain relief for most patients.

Partial Knee Replacement

Unicompartmental

A partial knee replacement resurfaces only the diseased part of the knee while preserving the healthy bone, cartilage and ligaments. Because much of the natural knee remains intact, it can feel more “normal” than a total knee replacement in suitable candidates.

Patients often experience a quicker recovery, less pain in the early phase and more natural knee movement. It is most appropriate when arthritis is confined to one compartment, the cruciate ligaments are intact and the knee is not severely stiff or deformed. If arthritis later develops in other compartments, conversion to a total knee replacement is straightforward.

  • Preserves healthy bone, cartilage & ligaments
  • Faster recovery than total replacement
  • More natural knee feel post-surgery

Patellofemoral Joint Replacement

Kneecap specialist

A patellofemoral replacement is a specialist partial replacement for isolated wear of the kneecap and its groove — typically causing severe anterior knee pain on stairs, slopes or rising from a chair. The operation resurfaces the back of the patella and the trochlear groove of the femur, leaving the rest of the knee untouched.

Because the major ligaments are preserved and only the affected area is treated, the knee can feel very natural and recovery is often quicker than with a full replacement. This option is only suitable when the remainder of the knee is healthy, so careful assessment and imaging are essential.

  • Targets anterior (front) knee pain specifically
  • Natural feel — major ligaments left intact
  • Requires healthy surrounding compartments

Choosing the Right Operation

Selecting between total, partial and patellofemoral knee replacement depends on which compartments are affected, the condition of the ligaments, degree of deformity, activity level and imaging findings. We always discuss all appropriate options and recommend the operation that provides the safest and most reliable long-term outcome.

I had a knee replacement surgery at Three Shires. The care I received was excellent, all the staff were friendly and made me feel at ease. The surgery went well , Mr Nasr and Mr Tanner were both welcoming and explained everything to me and made sure that I understood what was going to be done, even the after care was excellent. My time in Three Shires was very relaxing and the staff were there for you anytime night or day. I would highly recommend going there for your treatment.

Had a TKR at Three Shires Hospital on 7/11/25. Right from the very begining I was told what would happen and how it would feel. I had a nerve block, and the lovely anesthetist said he would also give me sedation. I can remember going into the pre-op room, the next I remember was waking up in recovery. I can highly recommend this hospital, was really looked after. I went in as an NHS patient in a private hospital. Due to have the other knee done soon.Thank you.

Had appointment with Mr salha. A consultant at three shires hospital. when we arrived we found plenty of parking.well signed posted. And well-maintained. On entering the reception it was well light and very cleen. The receptionist was very polite and directed me to were i needed to be. The dowas on time.well managed And explain everything in detail that need to be done. Escorted me to the x-ray department. Then when x-ray was sent to his computer.he proceeded to point out the problem things and explain what the next step should be. Left there with a hand shake and a feeling of reassurance I would highly recommend this hospital
